How to get Services

At Star of Hope, we believe everyone deserves the opportunity to live, work, participate, and thrive in their community. If you or someone you support is interested in receiving services from Star of Hope, the process starts with determining eligibility for Intellectual and Developmental Disability (I/DD) services through the State of Oregon.

Star of Hope is always accepting referrals and welcomes inquiries from individuals, families, guardians, Service Coordinators, Personal Agents, and other community partners.

Getting services can feel confusing. We want to make it easier.

 

Here are the basic steps.

 

Step 1: Find Out if You Can Get I/DD Services

 

First, you need to find out if you can get Oregon I/DD services.

​

In Coos, Curry, and Douglas counties, Community Living Case Management (CLCM) helps with this.

​

CLCM can help you:

​

  • Fill out an application

  • Get the papers you need

  • Find out if you qualify

  • Learn about services that may help you

  • Answer questions about the process

​

You do not need to know what services you want before you call.

​

Step 2: Meet Your Case Manager

​

If you qualify for I/DD services, you will have someone who helps you plan your services.

 

This person may be called a:

​

  • Service Coordinator, or

  • Personal Agent

 

They will talk with you about:

​

  • What is important to you

  • What kind of help you need

  • Where you want to live

  • What you want to do during the day

  • Work goals

  • Community activities

  • Other supports you may need

 

They will help you make a plan that is right for you.

​

 

Step 3: If you're interested in Star of Hope, tell them that is your choice. 

​

If you want to learn about Star of Hope, tell your Service Coordinator or Personal Agent.

 

They can send a referral to Star of Hope.

 

You can also contact us yourself to ask questions and learn about our programs.

 

Star of Hope is always accepting referrals.

 

 

Step 4: We Review Your Referral

​

When we get your referral, we learn more about you and what you need.

​

We may:

​

  • Review your information

  • Talk with you and the people who support you

  • Learn about your goals and interests

  • Talk about the services you are looking for

  • Invite you to visit a program

  • See if we have an opening that may work for you

 

We want to make sure Star of Hope is a good fit for you.

 

We may not always have an opening right away, but we are happy to learn about you and talk about possible services. 

 

Step 5: Start Services

​

If you choose Star of Hope and we have a service that is a good match, we will help plan your start.

We will work with you, your family or guardian if needed, and your Service Coordinator or Personal Agent.

We will make sure the right plans and paperwork are ready before services begin. Then we will work with you to support your goals, choices, interests, and needs.

 

Already Getting I/DD Services?

​

You do not need to apply again.

Tell your Service Coordinator or Personal Agent that you are interested in Star of Hope.

They can send us a referral.
